Brazil Type VITEM-III Visa

Brazil Type VITEM-III Visa Requirement:

To be eligible for a Type VITEM-III visa the purpose of the trip must be one of the following :

• paid participation in athletic or performing arts event.

1.Valid Passport:
Your signed passport must be valid and with sufficient blank pages for any visa stamps (Note: passport page 22, 23, 24 are not visa pages, and can not be used for visas).  .  The passport's expiration date must be greater than six months from the date of entry for the visa you are applying for. If your passport does not meet the requirements, you must renew your passport or obtain a new one.
Note: You need to send your original passport to obtain the visa.

2.Complete Application:
Download, print, fully complete, and sign one copy of the application form.

3.Photographs:
You must provide one 2"x 2" passport photograph. Machine or home photographs are not acceptable.

4.Work Authorization:
Work authorization by Ministry of Labor is required prior to visa application

5. Supporting Documents:
Letter from sponsoring organization, clearly stating the precise nature of the event

6.Order Form:
Download, print, fully complete the order form.

7.Payment Methods:
You may pay by personal check, business check or money order, made payable to: Visa Express.

8.Total Fee:
You may pay the total fees with one check, the total fees includes: Consulate Fee, Our Service Fee, Return Mail Fee. Please check the following table for the consulate fee, and service fee.
Our Return Mail Fee is $ 19.00 for Fedex Next Day Service.
