If you fall within one or more of the following criteria,
you must apply for a new passport:
- You have never had a passport.
- You had a passport but it expired
more than 5 years ago.
- You were a minor under the age of 16 when
your passport was issued and are now over 16 years of age.
To proceed with obtaining a new passport through Visa Express
Service, please submit the following documents to an official
Witnessing Clerk (This clerk can be an authorized representative
located at a United States Post Office or the County Court Clerk).
1. Completed US Passport Application (DS-11)
Click on the link at the bottom of this section to access the electronic DS-11.
You must fill out this form electronically and then print one copy. Click on the "create form" button to generate the completed form. Please do not sign the form until you appear in front of your local passport Acceptance Agent.

2. Proof
of U.S. Citizenship (one of the following documents)
- A certified United States birth certificate
- An expired United States passport
- Original naturalization papers
3.Two (2) identical passport-type
photographs
Must be official passport-type only and approximately 2" x 2"
with a clear background. Photos must be sufficiently recent
to be a good likeness and need not be signed. Glossy or matte
finish photographs are acceptable. Machine or home photographs
are not acceptable. Do not wear a hat or any item that covers
the face (except prescription glasses). Face forward only;no
side shots or profiles will be accepted.
4. Proof
of identification issued by the government or state
- State issued driver's license (at least 6 months old) bearing
a photo
- Military I. D. bearing a photo
- Government I. D. bearing a photo
5. Letter
of Authorization
Download and complete the Authorization Letter. This letter
will authorize Visa Express to submit and obtain the new passport

6.Order Form

7.Proof of Departure
A copy of airline ticket or an original itinerary will be needed. (Proof
of departure will vary according to the U.S. Passport Office
issuing your passport.)
8.Fees
and Payment
Government Fees:
Make one check payable to "US Department of State" for $127
(this is the government processing fee).
Make second check or money order in the amount of $30 payable
to the Post Office or Court House.
Our Service Fee:
Our Service Fees:$69.00 for Regular Service(8 - 12 business day).
Our Return Mail Fee is $ 19.00 for Overnight Service .
You may pay the service fee with return mail fee with one check, total $88, made payable to: Visa Express.
9.Authorization
Bring all original materials to a Passport
Acceptance Facility and notify the agent you are using an
expediting service to "hand carry" your application. As stated,
please present all above listed items to the authorized representative
for approval. You will then sign the passport application form
in the presence of the representative who will witness your
signature and select the following items from the above list
to place in an envelope(The representative will then seal the
DSP-11, photos, proof of citizenship, travel documents, authorization
letter and the $127 check in an envelope marked "To be Opened
by U.S. Passport Office Only". Do not open this sealed envelope
or the application will be invalid ) : Important: Make
sure the agent returns the sealed envelope to you. If the agent
does not feel at ease about releasing the envelope, suggest
that he/she refer to page 31 or 44 of their Passport Agent's
Manual, "Hand-carrying of Executed Applications".

10. Ship Your Package
