Rush Passport Amendment Service
If your passport was issued more than one year ago, you can not make name change on your currenct passport, and you should use the passport renewal instruction.
To proceed with obtaining passport amendment through
Visa Express Service, please submit the following documents to
us:
1.
Completed Passport Application
(DS-5504)
You have a choice of downloading
the application form or going to a Passport Acceptance Facility
to pickup the application form.
2.
One of the following items (item may vary according to reasons
for name change):

Original or certified marriage license

Original or certified divorce decree
-
Documentation showing use of professional
name for business or entertainment reasons
3.
Letter of Authorization
Download and complete the Authorization Letter. This letter will
authorize Visa Express to submit and obtain the new passport on
your behalf. Download Authorization Letter.
4.Order Form
Download and complete the Order Form. Download
Order Form.
5.Proof of
Departure
Airline ticket or an original itinerary will be needed. (Proof
of departure will vary according to the U.S. Passport Office issuing
your passport.)
6.Fees and
Payment
Government Fees:
Make one check payable to "US Department of State" for $60 (this
is the government processing fee).
Service Fee:
Our Fees:$99.00 for Rush Service(4-7 business days).
Our Return Mail Fee is $ 19.00 for Overnight Service .
You may pay the service fee with return mail fee with one check, total $118, made payable to: Visa Express.
7. Ship Your
Package Using Fedex or UPS only. Do not use U.S. Post Office Express Mail !!!
Place the following documents in a FedEx or UPS Express package.
- Application from (DS-5504)
- Valid passport
- Two passport photos
- Proof of of name chnage
- Authorization letter
- Proof of departure
- Check or money order for $60 to U.S. Department State
- Payment for $99 service fee
- $19 payment for over night return mail fee
