You should apply for a new passport if you answer Yes to any of the following:
* You are applying for a US passport for the first time.
* Your expired US passport is not in your possession.
* Your previous US passport has expired and was issued more than 15 years ago.
* Your previous US passport was issued when you were under 16 and you are now over 16.

You should do a passport renewal if you answer Yes to all the following questions:
* You still have and can submit your most recent passport.
* You were over the age of 16 when the passport was issued.
* You received your most recent passport within 15 years.
* The current passport is undamaged

If your passport was lost, stolen or damaged and it would still be valid then you are applying for lost/stolen passport.

Special requirements for children under age 14 minor children must apply in person

If your name has been changed since your passport was issued, then you need to submit proof of the name change such as court order, marriage certificate or other satisfactory evidence.
